The Biology of Anorexia Nervosa-Based on Treasure’s Model

신경성 식욕부진증의 생물학-Treasure의 모델에 근거하여

Abstract

Anorexia nervosa is a physical and psychosocial disorder that occurs most frequently in adolescent girls and young adult women. A decade ago, anorexia nervosa was rare outside of the developed western countries. However, it is now becoming a common clinical problem among young women in Korea. It is not enough to merely focus on relieving patients from the symptoms of "not eating," which is a practice that has been adopted by some forms of hospital care. The evidence base to guide treatment is limited. Nevertheless, there is the hope that a better understanding of the factors that play a role in the initiation and maintenance of disordered eating behaviors may be lead to more sophisticated treatments. This review aims to look beyond the overt "not eating" phenotype of anorexia nervosa and considers eating disorder endophenotypes based on Treasure's model. The first part of the review sets the basis for a framework of potential eating disorder endophenotypes. A description of the evidence of disordered eating behaviors as well as the clinical and psychopathological features associated with the central control of appetite follow. Finally, we describe how endophenotypes can be translated into treatments.

일본, 대만, 중국 등 우리나라와 비슷한 문화의 극동 아시아권 국가에서는 최근 섭식장애의 유병율이 급격히 증가하는 추세이다.1 섭식장애의 유병율은 연구에 따라 다소 차이가 있지만 많게는 젊은 여성 인구의 10%에 달한다고 보고 되었다.2 아직 국내에서 충분한 표본을 대상으로 한 섭식장애에 관한 정확한 유병율 조사는 없었지만, 한국 여성들에서 극동지역 타 국가에 비해 외모에 대한 불만족(body dissatisfaction)이 월등이 높다는 보고1나 비정상적 다이어트 열풍을 고려할 때 국내에서 이 병의 유병율이 급격히 증가하고 있음을 짐작케 한다. 최근 국내 대중매체에서 신경성 식욕부진증(거식증) 문제를 자주 다루지만, 제대로 알려주기보다는 대중의 호기심만을 자극하고 문제의 본질을 오도하는 경우가 많다. 기아 상태의 패션 모델 사진은 매력적으로 포장되어 메스컴과 광고판을 어지럽히는 반면, 진료실에 오는 거식증 환자와 가족으로부터 황폐한 삶과 죽음에 대한 염려, 균열되어 가는 암울한 가정을 보게 된다. 
   본 종설에서는 섭식장애의 표현형(phenotypes), 내적표현형(endophenotypes), 유전자형(genotypes)을 비롯하여 뇌생화학적 측면에서의 새로운 이론을 정립한 Treasure 교수의 가설을 소개하고자 한다. 본 종설은 거식증의 피상적인 면을 다루는 데에서 한걸음 더 나아가 이 병의 생물학적 이해를 위한 뇌 기전에 초점을 맞출 것이다. Treasure 교수가 제시한 내적표현형이라는 기본구조를 이용해 거식증의 생물학적 체계를 설명하고, 정형화된 치료법을 대체할 수 있는 새로운 맞춤 치료법에 관한 그녀의 아이디어를 국내 독자에게 정확하게 전달하여 이 병의 본질에 관한 이해를 돕고자 함이 본 종설의 목적이다. 거식증의 다양한 생물학적 원인론에 관해서는 Treasure 교수의 다른 논문들을 참고할 것을 권한다.3,4,5,6,7,8

본     론

거식증의 임상특징-표현형
  
거식증과 폭식증이 다른 질환과 뚜렷이 구분되는 핵심병리는 두 질환 모두 체중과 체형의 가치를 과대평가하고 이에 영향을 받는다는 것이다. 이는 남녀 모두에서 공통이다. 그 결과 나타나는 거식증의 핵심적인 표현형은 식사행태 변화와 체중 감소이다. 전통적으로는 거식증의 행동이상을 불충분한 열량 섭취라고 정의한다. 이는 굶기로 표현되는데, 체중을 줄이겠다고 결심하고 지속적으로 이를 추구한 결과 어느 정도는 성공한다. 거식증 환자들은 이런 행동을 문제라고 인식하지 않으며 저체중을 성취의 결과라고 여기지 고통이라 생각하지 않는다. 따라서 변화에 대한 동기가 적다. 하지만, 환자의 대략 1/3은 극단적 과식(즉, 폭식) 삽화를 보인다. 또한, 식욕을 억누르거나 폭식을 상쇄하고자 하는 다양한 보상 행동들(예를 들어, 운동, 구토, 하제 남용)도 임상적 특징이다. 환자의 40% 이상이 과다한 운동을 하는데, 이러한 행동은 강박적 행동과 관련이 있다.9,10 거식증과 강박증상, 위험회피(harm avoidance), 억제 성향(inhibited personality) 등은 관련이 크다.11 또한 이들은 우울, 불안, 이자극성, 기분변화, 집중력 감퇴, 성적 흥미의 상실을 보인다. 이러한 특징들은 체중감소에 따라 악화되고, 체중회복에 따라 호전된다. 대부분의 환자는 자신이 속한 문화에 거식증적 행동을 잠입시켜 합리화하고, 이러한 행동은 시대와 나라에 따라 다양한 방식으로 표현된다. 현대 사회에서는 체중과 체형으로 거식증의 임상양상을 설명하는 경우가 일반적이나, 이 병이 사람들에게 인식되기 시작하던 시대의 초기에는 다양한 방법으로 현재와는 달리 설명하였다. 본 종설에서는 이 질환의 유전자형 및 내적표현형부터 기술하겠다. 그런 다음 이 병의 생물학적 특징에 대해 기술하고자 한다.

유전자형
  
섭식장애는 가족 내 상대적 위험도가 10 정도로 가족 내 응집되어 있다.12 쌍생아 연구에서 유전력은 33%부터 84%에 이르는 것으로 나타났다.13 제한형 거식증에서 가족내 1번 염색체와의 연관 및 세로토닌(5-HT) 기능을 조절하는 유전자의 이상이 발견되었다.14 거식증 가족 내에서 강박 성향 또한 이러한 1번 염색체의 부위와 관련되어 있다.15 그러나, 유전 연구들은 크기효과(effect size)가 적고, 적은 효과의 많은 유전자들이 관여할 수 있다. 더군다나, 대부분의 연구에서 표본수가 크지 않으며, 다른 연구에서도 결과가 재현된 경우가 드물다. 

중간표현형(Intermediate phenotype) 및 내적표현형(Endophenotype)
   거식증에서 보이는 특징적인 강박성향(완벽주의 및 실수에 대한 우려 포함)은 어린시절16에도 나타나고, 회복 후에도 남아있으며,16,17,18 가족 내에서 이러한 특성을 공유17,18,19,20,21 하기 때문에 중간표현형이 될 수 있다. 쌍생아 연구는 이러한 특성 구조가 어느 정도는 유전적임을 시사한다.22 Treasure 모델에 의하면 이러한 특질의 기저에 과제이동능력(set-shifting) 및 중심응집력(central coherence) 등 정보처리과정의 이상이 있다.

다양한 정신질환의 내적표현형-과제이동능력 
   최근 거식증의 원인모델에서 과제이동능력의 이상이 핵심요소로 대두되었다.23 Tchanturia와 Treasure 등은 섭식장애를 대상으로 한 그간의 신경인지기능에 관한 연구들을 포괄적으로 검토한 결과, 수많은 검사 중 특히 과제이동능력의 이상을 확인하였다.24 한 걸음 나아가, 과제이동능력은 섭식장애의 내적표현형 진단기준을 만족한다. 첫째, 과제이동능력 이상의 크기 효과는 급성기와 회복기에서 서로 비슷하다. 둘째, 이러한 특성은 이환되지 않은 가족에서도 나타난다.25
   그러나, 과제이동능력 손상은 섭식장애에서만 특징적인 것은 아니며, 기분안정기의 조울증 환자26,27 및 그 가족에서도 나타난다.28 또한 정신분열병 환자의 가족에서도 관찰된다.29 과제이동능력이상은 보상 예민성(reward sensitivity)의 변화 및 도파민 체계와 관계가 깊다.29,30,31 종합하면, 과제이동능력 이상은 다양한 형태로 표현되는 정신질환의 발병 위험을 높이는 내적표현형일 가능성이 있다.

또 하나의 내적표현형-전체 대 세부의 왜곡(A bias to detail over global processing;central coherence)
  
Treausre 등은 중심응집력의 이상을 거식증에서의 또 하나의 내적표현형으로 제시하였다. 거식증 환자는 분석적, 세부적 전략을 요구하는 친숙한 도형 찾기(the Matching Familiar Figures Test)32와 잠입도형 검사(the Embedded Figure Task) 등33의 수행능력이 앞선다. 거식증에서 시각적 기억능력 및 중심응집력의 빈약함은 레이복잡도형검사(the Rey-Osterrith Complex Figure Test)에서 관찰할 수 있다.34 이는 빈약한 기억체계에 전체 도형 하에서 세부구조를 구별하지 못한 결과다. 중심응집력 약화란 국지적 정보처리를 과다하게 하는 과정에서 빚어지는 전체에 대한 왜곡을 일컫는다. 자폐나 아스퍼거 장애에서도 중심응집력이 약화된 것을 볼 수 있는데,35,36 이는 그들의 일차가족에서도 관찰37 되기 때문에 중심응집력 약화는 자폐 스펙트럼장애의 내적표현형이 될 수 있다. 이런 것들을 고려하면, 중심응집능력 약화는 거식증 발병의 위험을 증가시키는 비특이적 내적표현형일 가능성이 매우 높다. 

학습 및 조건화에 관련된 내적표현형
   Strober38는 거식증의 핵심요소로서 놀람에 대한 조건화(fear conditioning) 반응의 이상을 제안하였다. 이 이상은 불안장애의 특징이기도 하다.39 그 외에도 보상이 주어지는 학습 과제의 수행 초기에 더 많은 실수를 범하는데, 이는 도파민 체계의 이상때문으로 생각된다.40 거식증 환자는 노력을 들여야 하는 학습과제는 잘 하는 반면, 흔히 할 수 있는 학습과제는 잘 하지 못한다.41 거식증 환자가 흔히 환경의 요구에 맞추지 못하여 삶에 실패하게 됨을 이러한 학습능력의 이상으로 설명할 수 있다.

정서적 내적표현형
  
새롭게 대두되는 내적표현형으로서 정서적 내적표현형이 있다. 보상(;상) 추구체계 뿐 아니라 공포-불안(;처벌) 체계의 이상 또한 앞서 기술한 대로 섭식 장애의 정서적 내적표현형42의 표식자가 될 수 있다. 섭식장애에서, 자율신경계 반응 및 스트레스계 반응이 비특이적으로 나타날 수 있다. 예를 들어, 스트레스 검사에서 섭식장애 환자는 심박동수 증가나 코티졸 분비 반응을 보이지 않는다.43 거식증 환자는 아이오와 도박과제(the Iowa Gambling Task)에서 고위험성 선택 시에도 피부 전도반응이 증가하지 않는다.44 이러한 시상하부-뇌하수체-부신의 스트레스 체계의 반응 이상은 회복 후에도 지속되며45 거식증을 설명하는 대표적인 모델이 되어 왔다.46
   거식증에서 정서 등록(registration of emotion)의 이상을 보일 수 있지만, 이에 관해서는 연구결과가 일관되지 않았다.47,48,49 인지발달수준을 측정하는 도구를 이용해 정서처리의 정도를 측정했을 때, 거식증 환자에서 다른 사람의 정서를 인식하는 능력이 결핍되어 있다.51 일부 소집단에서는 다른 사람의 생각을 추론하는 마음의 이론 과제(the Theory of Mind Tasks)에서 수행능력이 결핍된 양상을 보였다.5 향후 섭식장애 환자에서 정서적 표현형을 정의하기 위해서는 더 많은 연구들이 필요하다.

정상 섭식행동의 생물학
  
다음은 중추에서의 섭식행동 조절을 두 핵심 요소인 항상성(homeostatic)과 쾌락적(hedonic) 측면으로 구분하여 설명하겠다. Fig. 1에는 중추의 해부구조와 신경화학 및 중추로의 섭식 신호 전달에 관여하는 주요 말초신호가 나와 있다. 섭식행동의 항상성 조절에 관여하는 물질인 렙틴이 매개하는 말초와 중추간 연결의 결함이 나타나면,52 시상하부 및 그 이상의 중추 체계에서 연속반응이 일어난다.53 최근 쾌락측면에서 섭식행동의 생물학적 체계를 설명하고자 하는 연구가 활발히 진행되고 있다.54,55,56 이 체계에는 두 가지 요소-원함(wanting)과 좋아함(liking)-가 있다. 원함, 열망, 음식에 대한 극단적 갈망 등의 보상체계에는 도파민 회로가 관여한다. 충동만족을 위한 반사(consummatory) 행동, 선호(liking) 행동, 쾌락(pleasure) 행동 등에는 오피에이트(opiate)계와 카나비노이드(cannabinoid)계가 관여한다.57,58 보상체계를 통제하는 주요한 요소는 항상성이며, 환경과의 상호 작용의 경험에 의해 보상체계가 형상화된다.59 한편, 쾌락체계는 식욕 촉진 뿐 아니라, 개체의 행동 선택을 지배하는 포괄적인 부분이다. 예상되는 보상과 즐거움에 근거하여 행동이 결정된다.

보상체계의 생물학 
   개체는 보상체계에 의해 특정 사건(events)의 동기 및 정서적 의의를 가늠한 후 전후 맥락에 가장 맞춘 행동에 적응해 나간다.60 이 체계는 고도로 유연하며(highly plastic), 주변환경으로부터 학습한 것들을 각인시켜 오랫동안 지속해간다. 과거경험에 근거해서 강화 요인(reinforcer)이 어떤 것인지에 대해 판단이 내려지면, 당장 어떻게 행동할 지를 결정하도록 영향을 준다. 이 체계의 핵심 과정은 새로운 것 추구(novelty seeking);충동성(impulsivity);민감성(sensitivity);조건성(conditionability);보상과 처벌(reward & punishment);정서 조절 능력(the ability to modulate emotion) 등이다. 특히 청소년기는 이 체계가 성숙해 나가는 과정에서 결정적 시기인데,61,62 청소년기 동안의 경험들은 보상 체계 형성과정에 영향을 미쳐 이 체계를 왜곡시킬 수도 있다. 청소년기의 뇌는 약물을 포함한 보상과 스트레스에 취약하다.63 약물 남용은 보상체계를 조절하는 기본 체제를 변조시키는 것으로 생각된다. 이러한 이론들을 토대로 최근 섭식장애 연구에 중독분야가 응용되고 있다. 본질적으로 중독은 변하지 않는다는 것을 설명하기 위해 몇 가지 이론적 모델이 있는데 여기에는 "어두운 측면(dark side)" 모델64-정상적인 보상 신경회로의 기능은 줄어든 대신 항보상 체계(anti-reward system)라는 보완 기능이 지속되는-과 "과감작(supersensitivity)" 모델65,66.67,68,69,70이 있다. 이 이론들부터 섭식장애가 만성화되고 치료 저항성이 되는 이유와 과정을 설명할 수 있다. 

보상으로서의 음식
  
음식이 어떤 보상을 줄 수 있느냐는 개체간 많은 차이가 있다, 그렇지만, 종에 상관없이 동물들은 고지방, 단 음식 군을 본능적으로 좋아한다. 또한, 음식에 대한 반응에는 개개인의 성격도 관련이 있다. 보상에 대한 민감 성향은 맛 좋은 음식을 과식하는 현상과 관련이 있으며, 이는 체중 증가를 초래한다.71 개체간 보상 민감성의 차이는 음식 신호에 반응하는 보상 체계의 활성화 정도와 관련이 있다.72

비정상적 섭식 행동의 생물학
  
대표적인 비정상적 섭식행동인 극단적 소식 및 극단적 과식은 거식증 및 폭식증의 대표적 표현형과 밀접하게 관련된다. 극단적 소식 및 극단적 과식의 동물모델은 섭식장애의 병태생리를 규명하는 데 매우 가치 있기 때문에 이를 어떻게 섭식장애에 적용할 지에 관해 Treasure의 관점에 근거하여 설명하겠다.

극단적 소식
  
특정 종의 돼지에게 스트레스를 주면 식욕저하, 체중감소, 과다행동같이 거식증의 제반 특징을 동반하는 마른 암퇘지 증후군(lean sow syndrome)이 나타나며8 군집 생활에서 받는 스트레스는 동물에서도 체중감소를 초래한다.73,74 한편, 쥐에게 먹이를 제한하고 우리에 가두면 쥐가 쳇바퀴를 끊임없이 굴리게 되는 쳇바퀴 쥐 전형(running rat paradigm)을 볼 수 있다.75 이 모델에서 쥐는 쳇바퀴를 굴림으로서 음식에서 얻는 것 이상의 보상을 얻는 것으로 보이는 데, 이는 쥐의 생명을 위태롭게 할 정도이다.76,77 이러한 양상에는 신경화학적으로 오피에이트계 및 5-HT계의 이상이 관여한다.78 한편, 극단적 소식의 위험은 암컷에서 훨씬 높아 보인다.

극단적 과식
  
거식증은 극단적 소식으로 시작하나 많은 경우에 폭식으로 전개된다.79,80,81,82,83,84,85,86 폭식에 관한 동물 모델에는 한시적 영양부족, 소화경로에서 음식을 우회 시켰을 때(이는 임상적으로는 구토를 설명할 수 있는 동물 모델임), 간헐적 고열량 음식 공급, 스트레스 등의 요소를 포함하며, 이 요소들 일부 혹은 전부가 합쳐져서 폭식증 모델을 구성한다. 폭식증 모델은 뇌의 화학체계를 균열시키고, 도파민이나 내인성 오피에이트 및 카나비노이드의 분비체계를 와해시켜 이들 물질을 과다분비 시킨다. 더하여, 중독상황에서 보이는 보상회로의 활성화가 지속적으로 나타난다. 일단 폭식양상이 나타나면 맛좋은 먹이는 곧바로 폭식 재발의 도화선이 된다(이는 binge eating을 설명할 수 있는 동물 모델임).87 또한, 약물남용과 폭식은 상호 교차적이라 폭식을 습득한 동물은, 알코올이나 암페타민에도 쉽게 감작된다.88,89 이러한 동물모델은 극단적인 식사형태를 반복하는 폭식 구토형 거식증에 아주 타당하다.

거식증의 음식반응-중추해부학적 및 정신생리학적 관점에서
  
건강한 여성이라면 음식에 의해 보상 체계가 작동된다. 그러나, 거식증 여성은 전전두엽에서 예외적인 반응양상을 보이며90,91 이는 거식증에서 회복된 후에도 지속된다.90 또한, 거식증 여성은 음식 신호에 반응하는 놀람 반응이 비정상적으로 증가되어 있는데 이는 거식증에서 음식이 자율방어반응(automatic defensive system)의 원동력이 됨을 시사한다. 반대로, 건강한 여성에서 음식신호는 진정효과를 낸다. 거식증 환자는 음식에 대한 반응이 특이하다.92 이들은 음식신호에 대한 침 분비 반응이 적고,93 지방이나 단음식에 대한 선호도가 낮고,94,95 천천히 먹고,96 식사 중에는 피부전도반응이 커지는 자율신경계 각성반응을 보인다.97 이상의 결과들은 거식증에서 섭식중추, 그 중에서도 쾌락에 관여하는 중추체계가 와해되었음을 시사한다.

거식증과 뇌신경생화학 
   거식증에서 섭식행동 조절을 관장하는 중추신경 생화학 체계의 이상이 발견되었으나, 대다수의 연구들은 간접적인 측정 수단을 이용했다.98 최근 뇌스캔 기법을 이용해 병변을 국소화하려는 시도가 활발히 되고 있다.
   섭식장애에서 뇌 영상작업을 뇌 신경화학적으로 응용한 결과, 도파민계의 이상이 일관되게 보인다. 예를 들어, 거식증에서 회복된 정상 체중에서도, 선초체 앞부분의 D2 수용체에 도파민 동위원소 결합이 증가되어 있다.99 이는 체질량지수(거식증으로부터 비만에 이르는 일직선 상의)와 선조체 D2 수용체수 간 음의 상관관계를 보임과 상통하는 결과이다.70 그러므로 거식증에서 선조체내 도파민 강직성 분비의 저하는 섭식 동인(drive to eat)의 감소를 의미한다고 볼 수 있다. 이 같은 결과는 거식증에서 보이는 야윔 현상을 설명할 수 있다.100
   섭식행동에 관여하는 5-HT 체계는 처벌체계(혹은, 위험으로부터의 회피(harm avoidant) 체계)의 한 부분이다. 이 체계는 보상 체계와 반대로 작동한다. 거식증에서 회복된 후에도 5-HT 체계의 이상은 지속된다. SPECT(Single Photon Emission Computed Tomography)를 이용한 연구에서 폭식형 거식증의 급성기에 뇌 5-HT2A 결합이 감소되었음을 관찰하였다.101 또한, 회복된 후에는 뇌의 subgenuate cingulate 부위의 5-HT2A 결합 감소102 및 뇌의 보상관련 부위의 5-HT1A 결합이 증가되었음103이 관찰되었다.

거식증의 중추식욕 조절에 대한 결론
  
지금까지의 임상적, 신경과학적 연구 결과를 요약하면, 거식증 환자에서는 섭식과 관련된 동기 행동을 조절하는 체계에 이상이 있다. 이러한 현상은 회복 후에도 지속되며, 제한형 거식증과 폭식 구토형 거식증이 구분되는 부분이다.

생물학적 연구결과로부터 도출된 Treasure의 거식증 모델
  
청소년의 행동문제를 설명하기 위해 접근(approach), 회피(avoidance), 조절(regulatory)의 세 요소로 구성된 삼각 모델이 개발되었다.104 Treasure는 이 모델을 섭식장애에 적용하여, 회피행동으로 표현되는 벌에 대한 높은 민감성은 섭식장애의 범진단학적 특성이라 여겼고, 보상에 대한 민감성은 제한형과 폭식형을 구별하는 특성으로 보았다. 제한형 섭식장애 환자에서는 보상 민감성이 감소되어 있으며 접근행동이 적고, 폭식형 섭식장애 환자들은 반대 경향을 보인다. 과제이동능력의 취약, 분석적 접근, 학습부진 등 정보처리 능력의 결핍은 조절체계의 효율성을 감소시킬 수 있다.

거식증의 치료적 접근

현재까지의 치료
  
거식증의 치료에 있어 핵심은 네 가지 측면으로 나눌 수 있다.2 첫째, 환자가 치료의 필요성을 느낄 수 있도록 도와줌으로써 치료동기를 유지하게 하는 것이다. 이는 환자가 변화를 꺼려할 경우 특히 어렵고도 중요하다. 둘째, 체중 회복이다. 이는 영양실조를 되돌리고, 신체적, 정신적 상태의 전반적 호전을 도모할 수 있다. 체중회복은 입원, 낮병원, 외래 등에서 시행될 수 있는데 어떤 상황이 나은지는 의견이 분분하다. 셋째, 체형과 체중을 과대평가하는 생각, 식사행태, 전반적인 정신사회적 기능 등을 다루는 것이다. 이를 모두 달성할 수 있는 단일의 치료법은 없다. 청소년에서는 특히 가족치료가 효과적이며, 상대적으로 나이든 환자들에게는 인지치료가 효과가 있다. 이러한 치료들을 적용하는 데는 치료자들의 치료기술습득과 훈련이 필요하다. 이런 치료들은 외래에서도 성공적으로 수행될 수 있다. 넷째, 강제 치료(compulsory treatment)이며, 이는 극소수에 해당된다. 환자가 원하는 것과 좋은 치료를 받을 권리를 조화하는 것은 어려울 수 있다. 따라서 강제치료는 보호자의 동의 및 법적인 허락 하에 수행하는 것이 바람직하며, 결코 쉽게 여겨서는 안 된다.

생물학적 연구결과의 치료 응용
  
이상에서 설명한 Treasure가 제시한 거식증의 생물학적 모델은 기아 상황의 거식 삽화의 반복과 보상 체계 및 쾌락 체계의 영구 손상, 부모의 통제 등의 발달적 요소 및 고열량 음식에 대한 간헐적 접근과 폭식삽화 출현, 폭식삽화와 보상체계의 영구적변화에 관한 설명이 가능하다는 점에서 매우 타당한 모델이다. Treasure는 이 모델에서 설명하는 보상체계의 조형성(plasticity)과 재설정(resetting)을 치료에 적용3,105하였다. 이러한 치료적 요소에 관해서는 Fig. 2에 잘 나와 있다. 이 치료 방법을 조기에 적용할수록 효과가 뚜렷하다는 연구 결과는 거식증의 경과에 따른 보상 체계의 영구적 변화에 근거하여 설명할 수 있다.105 치료에는 다음 내용을 포함한다.
   건강한 식사, 즉 충분한 양, 다양한 음식, 사람들과 함께 하는 식사 습관을 다시 배워나가는 것을 강조하는 치료
   정보처리의 왜곡을 바로잡는 치료
   접근-회피(approach-avoidance) 체계 내 보상-정서(reward-emotion)의 균형을 도모하는 치료
   가족의 과잉정서표출, 과잉보호, 증상에 대한 순응을 줄이게끔 가르치는 가족치료법106

결     론

   거식증 환자의 뇌 구조의 생물학적 체계를 이해하고자 진행된 여러 연구들을 종합한 결과, Treasure는 거식증의 주요 위험 요인으로서 과제이동능력 결핍 및 중심응집력 약화라는 정서처리 및 정보처리 과정의 이상을 제시하였다. 또한 이 병의 만성화 및 유전성을 중독과의 관련성 및 보상체계의 영구적 변화 등으로서 설명하였으며, 이 분야는 앞으로 더 연구될 가치가 있다. Treasure의 모델은 회복이 어려운 거식증의 특성에 대한 설명이 될 수 있을 뿐만 아니라, 해당요소의 교정을 통한 맞춤치료로서 적용할 수 있다.
